8. Medical certificates

(commonly used forms)

SC2

Self-certification form used by an employee to provide their employer with details of sick absences of four or more consecutive days (SC1 for unemployed or self-employed).

Statement of fitness for work

Issued when the patient has been off work for more than seven days. It allows the doctor to advise one of two options:

  • Not fit for work: the patient is unfit for work and is advised to refrain from work for a stated period of time.

  • May be fit for work: the patient may not be able to complete all his or her normal duties or hours, or they may need some support to undertake these.

This statement replaced forms Med 3 and Med 5 in April 2010; Med 4, Med 6, and RM7 have also been withdrawn.

DS 1500

Issued when a person suffering from a potentially terminal illness (defined by Social Security legislation) where death as a consequence of the illness “can reasonably be expected within six months”. This allows rapid access to claiming personal independence payment (PIP) or disability living allowance (DLA) and attendance allowance (AA) under “special rules”.
